Individual Plan
1 person, monthly- One person, billed monthly*

2 people, monthly- Two people, billed monthly*

Family Plan
2 adults, up to 2 children- Two adults and up to two children, billed monthly*

Single Parent Plan
1 parent, up to 2 children- One adult and up to two children, billed monthly*
- The Single Parent Plan saves a family of three $28 a month off the regular rates.
- Unlimited office visits, calls, messages, with same or next-day appointments all included!

Some key details:
- New members and reactivations: $100 initiation fee per member or group, capped at $200 per family. Three-month minimum membership term.
- Quoted rates are based on monthly billing. Paying semiannually will save you 5%. Paying annually will save you 10%.
- You can add adults or children to a plan as long as they reside in the same household as the primary member. Adults are $91 per person, and children are $44.
- Schedule a free discovery call to add an additional member who has Medicare. They may be eligible for our Medicare Concierge plan.
- Childhood vaccinations are not included or available in-clinic.
- Our membership plans are not insurance products and do not cover care provided by unaffiliated third-parties. We recommend you have at minimum a high-deductible insurance plan or a cost share plan.
- Minor procedures are available when medically indicated. Not all procedures available at all times.
- Certain in-clinic medications, tests, and procedures may incur an additional charge. You’ll know the cost up-front before committing to care.
- Those with Medi-Cal / CenCal are not eligible for membership due to state regulations.
